Information you choose to provide
Consultation and privacy forms collect your name, email address, optional message, consent and submission time. The assessment interest form collects your email and consent. We may record campaign labels from the page URL to understand how you found the site. Please do not submit diagnoses, medication doses, laboratory results or detailed medical information.
How information is used
Submissions are used to respond to your request, manage assessment interest, maintain records of consent and help prevent repeated or abusive submissions. Submitting a form does not sign you up for the Beehiiv newsletter. A separate newsletter connection and consent would be required.
Storage and access
Form submissions are stored in the website’s hosted database. Authorized site operators can access them. Hosting and infrastructure providers process information as needed to operate the site. When email notifications are enabled, your inquiry is also sent to hello@optimalintegrativehealth.com through Resend, our email delivery provider. Email providers process the message for delivery. We do not sell form submissions or use them for advertising audiences.
Cookies and analytics
This version does not install third-party advertising or analytics scripts. Hosting services may process technical information necessary for delivery, security and, in the private preview, access control. In-page interaction events contain event names rather than your form entries and are not sent to a third-party analytics service.
Retention and your choices
You may ask to access, correct or delete information you submitted, or withdraw consent for future assessment updates, using the privacy request form below. Requests may require identity verification. Information may need to be retained where required for legitimate recordkeeping or legal obligations. A specific operational retention schedule will be established before public launch.
External services
External links lead to services governed by their own policies. An appointment scheduler, newsletter provider or CRM is not currently connected to these forms. Email notifications are separate from newsletter subscriptions and appointment scheduling.

This site is intended for adults. It is not a medical portal or a place to submit urgent or detailed health information.
